EMT & Fire Training Student Agreement
As a student applicant for an EMT & Fire Training Inc. course you must agree to the statements below. This EMT course agreement is also presented during the enrollment and payment process. At which time you must demonstrate your understanding and agreement to these statements by checking a box.
I am at least 18 years of age and have a high school diploma or GED equivilant.
I understand that I must have access to a computer with a highspeed internet connection.
I understand that I am required to attend 5 days of in-person, hands on, EMT skills training and assessment at our facilities on Hayden Lake,Idaho.
I understand that if my cumulative grade for the EMT course is below 80% following the final examination it will be considered failure of the course and I will not be allowed to test for NREMT Certification.
I understand that I will be required to make and document 10 patient contacts on ambulance ride-alongs or in a clinical setting (emergency room). I further understand that EMT & Fire Training Inc. will assist me with contact information and the appropriate paperwork required to get these patient contacts during the course.
I understand that if I withdraw from the course within two weeks of initially accessing the course material I will be given a 70% refund. If I withdraw from the EMT course after having two weeks of access, no refund will be issued, but I will be permitted to continue and finish the EMT course.
I am free of all street drugs and medications not prescribed for me. I understand that I may be asked to complete a 5 drug screening prior to having any patient contact or using EMT & Fire Training Inc. facilities. Failure of the above mentioned drug screening can result in failure of the course.
I have never been convicted of a felony and agree to allow EMT & Fire Training Inc. to run a criminal history background check on me. I further understand that if a felony is found in my history I will be discharged from the course with no refund.
I understand that I am not registered for this EMT course until I have submitted this agreement and completed the registration and payment process that follows.
